Office Policies

To Make appointments: 

Please call 301-605-7468 between hour of 9:00 am to 12:30 pm  and 1:30 pm to 5:00 pm Monday through Friday. 

Please print Patient Registration Form, fill out, and bring it with you 15 minutes before  your initial appointment. 

We will make all effort to accommodate your appointment within 48 hours.

Office is closed on Christmas day ,New year's day , Memorial  day,Independence day  Labor day and Thanks giving day

Appointment Cancellations:

To cancel an appointment kindly give us at least 24 hours notice  

Referrals:

Some insurance plans require you to have an referral/ authorization  issued by your primary care physician to see one of our physicians. If your plan requires this, it is imperative that you bring it with you at your appointment.

It is to your benefit to secure your insurance referral prior to your visit to ensure your insurance benefits are maximized. 

Referral is a requirement by your insurance plan ,therefore  it will be necessary to re-schedule your appointment if you arrive without your referral.

Test Results:

Results are generally received in our office within 7-10 business days after tests have been performed. Our physicians review all reports and will discuss them with you at your next office visit.

Medical Records:

Original records are the property of the Practice and will be released based on a signed authorization. Per federal regulations, we require a signed Release of Medical Records prior to processing of requests. We require payment in advance. Processing will be completed within 5-10  business days from the date we receive your signed authorization and payment.

Forms Completion:

There will be a fee to complete disability forms associated with your care. We require payment in advance and please allow 5-10 business days for the forms to be completed.

Billing Inquiries:

Please call 301-605-7468  for any billing questions you may have. We will be happy to make every attempt to assist you. Please have all insurance information available at the time of your call.

Financial Policies:

To ensure care is available to all our patients ,  we require you to pay all co pay , deductible and balance in full at the time of visit. 

Payments may be made in the forms of cash, check, MasterCard or Visa.

We do not accept American Express .
